Player registration is a three-step process – you must complete all steps to complete the registration process!
Step 1:
Review and download the form for your physical and concussion paperwork. Please print out and bring to your doctor or facility for your physical examination.
Paperwork can be uploaded using the second link, below.
If you prefer, you may also turn in a physical copy to Lee Beasley (Team Player President) or Jacob Caplan (Team Player Vice President).

Step 2:
Complete the online registration process that allows us to gather all player contact information, home address, parent contact information, and e-mail addresses.
Step 3:
Go to the payment page and pay fees.
